ALLIANCE Self-Management Fund Reopens to Applications (Scotland)

The Health and Social Care Alliance Scotland (ALLIANCE) is offering grants of up to £150,000 for third and community sector organisations for projects supporting people in Scotland with long-term conditions to live well.

Two levels of funding are available:

  • Small grants of up to £15,000 for up to one year.
  • Large grants of up to £150,000 for up to three years.

Eligible projects must align with one or more of the following priorities:

  • Work that aligns with secondary prevention by working to embed self management soon after a long term condition or disability is identified, or early in someone’s unpaid caring role.
  • Work that seeks to address health inequalities while increasing the self management capacity of people with long term conditions, disabilities and/or unpaid caring responsibilities who experience these inequalities.
  • Work that develops the self management capacity of people living with long term conditions, disabilities, and/or their unpaid carers, whilst awaiting health or social care support.

All projects should start between 1 June 2026 and 1 December 2026.

The ALLIANCE is holding information sessions on 16 October 2025, 23 October 2025, 28 October 2025 and 11 November 2025. Applicants can register for these sessions on the ALLIANCE website.

Large grants have a two-stage application process.
